We specialize in providing services to manufacturing companies, distribution centers, sales, delivery and telecommunication centers as well as many
other areas.
All Onsite therapist are trained and educated in Osha Regulations for preventative care.
Action Onsite is a consulting preventative program, therefore we do not offer physical or occupational therapy.
Our therapists use their medical knowledge to explain how the body moves and functions either on a one to one basis or in a training classroom setting.
Our therapists use a hands on approach to educate your team members.

Sonia Lopez, OTR/L, HTC, PAM
President and CEO of Action Onsite, Sonia Lopez is a Bilingual Occupational Therapist. She is a graduate from Loma Linda University school of Allied Health Professions and has over 25 years of extensive experience in hands and the muscular skeletal system. Sonia specializes in industrial ergonomics. She has worked with the industry for over 15 years and is passionate about helping others. She enjoys working onsite with employees and training along side your employees. She is a regular lecturer with Loma Linda University. She enjoys bouldering, traveling and being with her family. |

Albert Vigil, PT
Albert graduated from Loma Linda University with his emphasis in Orthopedic Therapy. His experience has been in working at rehabilitation centers, hospitals, private practice, home health and contract work.
In 2010 Albert joined Action Onsite and covers our Kern Frito Lay site. He is married and has 3 children, enjoys the outdoors including mountains, beaches and sports. |

Amber Ineichen, MPT
Amber is a graduate of Loma Linda University with a Masters Degree in Physical Therapy. She has over 5 years of experience in the field of physical therapy and has worked in pediatrics, neuro rehabilitation, orthopedics and acute inpatient. Currently she is the ICU physical therapist at Redlands Community Hospital. Amber joined Action Onsite this year as one of our instructors at Stater Bros. In her free time she enjoys spending time with family and friends, traveling and being outdoors |

Audrey Thompson, OTR/L
Audrey graduated from San Jose State University with her B.S. in Occupational Therapy. She enjoys Neurological acute rehab, skilled nursing, hand therapy, out patient, pediatrics and in the industrial industry with injury prevention. She is a wife and mother of three. Currently is working for a home health agency while also working part time with Action Onsite covering our Manteca, CA, Frito-Lay site. Audrey has been with Action Onsite since 2005. She enjoys snow skiing in winter and going to the beach with her family in summer. |

Benita Kim, MS, OTR/L, HTC, CEES
Benita Kim is a licensed occupational therapist and ergonomics consultant. She received her B.S. in Sociology at the University of Illinois at Chicago and M.S. in Occupational Therapy at Rush University. Since 1999, she worked as an occupational therapist specializing in hand therapy and work-related injuries. After a few years of ergonomics experience at a hospital setting, she received her certification in ergonomics. In 2006, she began to provide independent ergonomics consulting with injury prevention to numerous employers in both office and industrial settings. In March of 2011, she joined the Action Onsite team for the Frito-Lay site in Concord, CA. In her spare time, she enjoys hiking, traveling, and dining at restaurants with her husband and playing golf with friends. |

Bernie Garcia
Bernie Garcia joined the team in 2009 and is a graduate from Long Beach University with a Bachelor of Science in Biology/Physiology and a minor in Chemistry. He also is a graduate from Loma Linda University with an Associate of Science degree as a Physical Therapy Assistant. He has over six years experience in the field ranging from outpatient clinics to inpatient ICU. Bernie also has seven years of manufacturing and distribution experience with Frito-Lay and Miller Brewing Company. Currently he is the ICU Physical Therapy Assistant for Redlands Community Hospital.

Beth graduated from Biola University in 1997 with a B.S. in Sports Physiology. She received her Masters degree in physical therapy from Azusa Pacific University in 2000. She currently works in an orthopedic physical therapy clinic in Whittier, CA and has recently ventured into women's health. She enjoys spending time with her husband and three young children. Beth joined the Action Onsite team in 2010 and is servicing the Kraft Foods site in Fullerton, CA. |

Christine Orr, PT
Christine Orr graduated from the Richard Stockton College of New Jersey with a Bachelor of Science in Physical Therapy. She has orthopedic experience in a variety of settings including acute care (inpatient and outpatient ), skilled nursing facility, home health, and industrial physical therapy. Christine is excited to be a part of the Action Onsite Team. She is presently servicing our Frito-Lay site in West Valley, Utah. |

Christy Mackiewicz
Christy Mackiewicz is a graduate from California State University at Long Beach with a BS in Physical Therapy in 1986. She has worked in CA, OR and WA in many different clinical environments. Her first love is working with children. She enjoys helping people make simple changes that can have a big impact on their wellbeing and productivity. Exercise should be pertinent to each individual's needs and easy to do. Christy lives in Battle Ground, Washington, and covers 2nd shift at our Frito Lay's Vancouver site. She is married, has twin teenage daughters and enjoys reading, hiking and as many sunny days as the Pacific Northwest will dole out. |

Debbie Holmes-Enix
Debbie Holmes-Enix is a licensed Occupational Therapist, who has education and training in work evaluation, functional capacity evaluation, job analyses, and ergonomics. She has practiced in industrial rehabilitation for twenty-nine years, and has been ergonomics consultant for the past twenty-five years. She graduated with high honors with a B.S. degree in O.T., and an M.P.H. in health education/administration from Loma Linda University. She is the owner of Y.E.S. – Your Ergonomic Solutions, an ergonomics consulting practice. She has consulted for a myriad of employers from Los Angeles to Blythe and Needles to El Centro. She absolutely loves working out, dancing in a Christian performance group, singing, and spending time with her family. She has partnered with Action Onsite, Inc. for a variety of ergonomic projects. It is her pleasure and passion to help individuals improve their work and lifestyle health for lifelong well-being. |

Hadeil Kotb, DPT, CSCS
In 2002 Hadeil Kotb graduated with her Doctor of Physical Therapy degree from the University of Southern California after completing her undergraduate degree in exercise science. Her primary experience has been in outpatient orthopedic hospitals / clinical settings, she is certified in strength and conditioning as well. Hadeil is passionate about educating others on prevention and self management skills. She has consulted and done seminars to Dental professionals. Hadeil joined Action Onsite in March 2011 and is servicing our Frito Lay Manteca D.C. site. She enjoys spending time with her husband and three young children. |

Heidi Jackson, OTR/L, HTC, CEES
Heidi graduated in 1982 from Loma Linda University with a B.S. is Occupational Therapy. She has practiced in the fields of acute hand rehabilitation, and work rehabilitation for some 20 years specializing in hand therapy. She is also a Certified Ergonomic Evaluation Specialist (CEES) and has been consulting with employers since 1985. She is certified in Pilates and Yoga instruction and consults privately and in local physical therapy practices. Heidi has been a team member with Action Onsite since 2007.

Jamie is a graduate from Mt Hood College with a degree in Physical Therapist Assistant. She is currently working with Back Up Therapy Staffing. She has worked outpatient Orthopedics for over 7 years. Jamie has been a team member with Action Onsite since 2008. She presently covers our Eugene Frito-Lay site. She is a wife and mother of 2 beautiful girls ages 20 months and 3 years. Jeannette Grace, MPT
Jeannette Grace graduated from Loma Linda University w/ her master in Physical Therapy after Sports Medicine Training in High School, working as a PT aide and getting her Massage Therapy certificate while attending SRJC for prerequisite classes. She loves the Outpatient setting, preventive therapy, training, and working in the industrial industry. Jeannette has been a team member with Action Onsite since 2007 and covers our Santa Rosa, CA, Frito-Lay site. As a hobby, she competes in the Scottish Highland Games. |

Jill Jandreau, MSPT
Jill has been a physical therapist in the Seattle area for over 9 years. Her areas of expertise include ergonomics, injury prevention, education, and neurology. In her spare time she enjoys rock climbing, hiking, travel, kayaking, and rollerblading. She is currently working on bringing wheelchairs to children in different countries. Jill has been a team member with Action Onsite since 2005. She presently covers our Frito-Lay Redmond, Everett, Federal Way, Olympia and Mount Vernon sites.

Jodi Decker, PT
Jodi Decker is from the Midwest and has been practicing PT for over 26 years. She presently works at Balanced PT. She has recently joined our Action Onsite team and covers our Frito-Lay Vancouver, WA site. Jodi has extensive experience in orthotic fabrication and in treatment of the full spine. Jodi’s tries to equip each person she sees with tools they can use to successfully reduce their pain and gain better function. Jodi is married and a mother of four. She loves to read, garden and play ultimate Frisbee. |

John Parr PT, CMPT
John Parr is a graduate from the University of Washington where he earned his degree in Physical Therapy. He has achieved advanced certification in manual therapy through the North American Institute. John specializes in manual therapy and is also a certified golf instructor.
His passion is in helping people reach their goals through highly personalized service and care.
John has been the director of Therapeutic Associates since 1998. He has recently joined the Action Onsite team for our Frito-Lay sites in the Portland and Vancouver areas. John enjoys spending time with his family. He also loves playing sports of all kinds, including softball, racquetball, golf, and tennis. John is an avid runner and has competed in both the Portland and Boston Marathons. |

Judy Johnson, PT, OCS
Judy graduated from Cal State University Long Beach in 1976 with a Bachelors of Science degree in Physical Therapy. She worked in a hospital setting for 12 years then transitioned to an outpatient physical therapy clinic in 1989. She has a strong orthopedic background and received her Orthopedic Clinical Specialist credential in 2006. She has been involved in all aspects of industrial rehab since 1993, including early intervention, functional capacity evaluations, pre-work screens and ergonomic assessments. Other areas of expertise include Women’s health issues. In August 2011 she joined the Action Onsite team assisting in servicing our Rancho Cucamonga Frito Lay site. Her leisure activities include: hiking, (she recently climbed Half Dome in Yosemite with her husband), reading, fine dining, gardening, and of course spending time with her family. |

Kara Murphy
Kara has been working as an occupational therapist for nine years, in California, Texas and now Oregon. She obtained both her undergraduate and graduate degree in occupational therapy at the University of Southern California. She has enjoyed working in a variety of settings in pediatrics, home health, inpatient acute and outpatient rehabilitation, but specializes in upper extremity orthopedics and neurological rehabilitation. When not enjoying her work as an OT, Kara loves spending time with her daughter and husband, as well as cooking, eating and reading cookbooks. |

Katie Pecson
Katie Pecson graduated in 1988 with a MS Degree in Molecular Biology and received her MS Degree in Physical Therapy in 1995. Katie has practiced in a variety of settings as a Physical Therapist including pediatrics, acute rehab, skilled nursing, inpatients and outpatient orthopedics. Katie feels fortunate to be in a field where one is rewarded by helping people achieve their personal goals (e.g. return to sport, learning to walk again, or working pain free). Katie is a team member with Action Onsite and covers our Alameda, CA, Frito-Lay site. In her spare time, Katie enjoys spending time with family, and getting outdoors for some exercise with a friend or her dog. |

Linda de Haas, MPT, OCS, CHT
Linda has been in Physical Therapy for over 20 years. She graduated from Cerritos College as a P.T. Assistant in 1985; she then received a Bachelor of Science from Cal. State Long Beach in 1990 with an emphasis in community health education. Linda received her Masters degree in Physical Therapy from Samuel Merritt College in 1995, and became board certified as an Orthopedic Clinical Specialist in 2001. Linda was the hand therapy fellow at Kaiser 2007-2008 and became a Certified Hand Therapist in 2008. She is presently working at Professional Physical Therapy Associates in Whittier, CA. Linda became an Action Onsite team member in 2009 and is servicing our Kraft foods site. Linda is a lecturer in the Kaiser Hand Therapy Fellowship Program. |

Lisa Harbin is an occupational therapist who obtained her Bachelor of Science degree in occupational therapy in 1991 from San Jose State University. In 2005 she also has her Master of Arts degree in education with her teaching credential from Point Loma Nazarene University in Bakersfield, CA. She has been a clinician for over 18 years. Lisa’s experience encompasses a variety of settings including acute hospital, outpatient and inpatient rehabilitation, skilled nursing, home health, and elementary school sites and in the industrial industry. Lisa has been a team member with Action Onsite since 2004 and is servicing our Kern Frito-Lay and Bolthouse Farms, Inc. sites. |

Mary received her undergraduate degree in Psychology from the University of California, Irvine in 1982 followed by her Masters degree in Occupational therapy from San Jose State University in 1986. Her career as a therapist has included emphasis on return to work programs, ergonomic interventions, and rehabilitation of hand injuries. She has been a certified hand therapist since 1992. When not working, Mary enjoys spending time with her two daughters and husband, relaxing at the beach, cooking and walking half marathons. |

I am a WA native who graduates this coming August with my Doctorate of Physical Therapy from Arizona School of Health Sciences. I received my Bachelor’s of Science from Northwest Nazarene University where I played baseball. My wife of 4 years and I then moved to sunny Scottsdale, AZ for the past 3 years where I did my graduate school. I am an avid sports fan and enjoy spending time wakeboarding. I have taken a job with Battle Ground Physical Therapy, where I will get to practice my passion of orthopedic physical therapy. |

Sharon Pasquinelli, MS, OTR/L, HTC, PAM
Since 1995, Sharon has provided ergonomic services to a wide variety of high tech and manufacturing companies. She has been a principal evaluator for office and non-office ergonomic interventions, including chair fittings, equipment recommendations and injury/discomfort root cause analysis. Sharon has developed curricula and taught injury prevention courses including office, non-office and back injury prevention and 'train the trainer' programs. Sharon is the recipient of the Star Performer Award for providing exceptional customer service and she has published several chapters in leading occupational therapy texts. She is bilingual in several languages. |

Sharon Snarr has been working in physical therapy for over 12 years. She is a graduate of from Loma Linda University. She is also the Owner of Snarr Tech providing pre-employment physicals and FCE's and is a WorkSTEPS certified Clinician with experience. Sharon has been a team member with Action On-Site since 2004. She presently is consulting at the Frito-Lay Modesto Manufacturing plant. Working with Action On-Site has been a perfect fit for Sharon as she is able to continue working in a field that she loves as well as staying at home with her growing family. |

Stacy Jensen has been an Occupational Therapist for 26 years. Stacy graduated from The University of Southern California in 1982. Her specialized skills are in the areas of Hand Therapy and Rheumatology. She became a Certified Hand Therapy in 1992, and is trained in the treatment of Lymphedema. Stacy strives to help her client's achieve a balance of work, rest and play within their disability and or disease process. Stacy has been working with Action Onsite since Fall of 2008 at our Cardinal Valencia site. |

Sue has worked in the medical field since 1991. She has worked as a surgery scheduler/ back office assistant. She then joined the Rehabilitation Technology Works team in accounts receivable, billing and collections for nine years. In 2006, Sue joined the Action on site team as the administrative assistant. She enjoys hiking, traveling and spending time with her family. |
